Wheelchair Pressure-Reducing Pads Policy 

Purpose: 

  • To support the bony prominences of the ischial tuberosities. 

  • To reduce pressure and prevent skin breakdown. 

  • To distribute pressure evenly over the surface of the buttocks. 

  • To maintain adequate circulation. 

  • To relieve pain and discomfort. 

Equipment: 

  • Foam pad 

  • Gel pad 

  • Air pad 

  • Combination of the above 

  • Appropriate pad cover 

Note: Do not use donut-type devices. 

Procedure: 

I. Placement and Preparation: 

  • Any type of pad used should support and hold the bony prominences suspended above the seat of the chair. 

  • Place the pad on the wheelchair and cover it with the appropriate cover. 

II. Assisting the Resident: 

  • Assist the resident into the wheelchair, using proper transfer techniques. 

  • Ensure that clothing or protective cover is not wrinkled under the resident. 

  • Make sure the resident’s clothing covers their back so that their skin is not in contact with the plastic backrest. 

  • Cover the legs with a lap robe if desired by the resident. 

  • Pad wheelchair arm and leg rests if necessary. 

III. Positioning and Comfort: 

  • Reposition the resident at least every two hours and toilet as necessary. 

  • Assist the resident to the desired area of the facility. 

  • Encourage and instruct the resident in proper procedures for safely propelling the wheelchair. 

  • Leave the resident in a comfortable position with the call light within reach. 

Documentation: 

Documentation may include: 

  • Date, time, and duration the resident was up in the wheelchair. 

  • Amount of assistance required to transfer from bed to chair. 

  • Type of pressure-reducing pad used to prevent irritation and skin breakdown.
