  • Stage 1 Pressure Injury is characterized by intact skin with a localized area of non-blanchable erythema, which may feel warmer or cooler compared to adjacent tissue. In darker skin tones, the injury may appear purple or maroon. The area might be painful or different in firmness compared to surrounding skin .

3. Coding Instructions

  • Enter the number of Stage 1 pressure injuries currently present. If there are no Stage 1 pressure injuries, enter '0' .

4. Coding Tips

  • Pay special attention to skin areas over bony prominences and conduct a head-to-toe assessment. Stage 1 injuries might be missed in individuals with darker skin tones; look for changes in skin temperature, firmness, and discomfort .
