Eye Irrigation

Eye Irrigation 

Purpose: 

To flush secretions, chemicals, and foreign bodies from the eye. 

Policy: 

This procedure is performed per physician’s order by a licensed nurse. The order will include the type of solution and frequency of the irrigation. 

Procedure: 

  1. Preparation: 

  • Assemble all necessary equipment and bring it to the resident’s bedside. 

  • Turn the resident’s head slightly toward the affected side. 

  • Supply the resident with a towel to wipe and/or an emesis basin to collect the excess irrigating solution. 

  1. Irrigation: 

  • Put on gloves. 

  • Flush the eye using a constant, direct, gentle stream of the prescribed solution, going from the inner to the outer section of the eye. 

  1. Completion: 

  • When finished, dispose of gloves and wash hands. 

  • Leave the resident in a comfortable position. 

  • Document the procedure and results in the Treatment Administration Record (TAR) and/or in the nurse’s note. 

Compliance and Documentation: 

  • Adhere to CMS guidelines and Requirements of Participation for Long-Term Care Facilities. 

  • Document the eye irrigation procedure, including the resident’s response and any observations, in their medical record. 

  • Note any abnormalities or difficulties encountered during the procedure. 

  • Regularly review and update techniques for eye irrigation according to the latest clinical best practices and regulatory standards. 

  • Provide training to staff on proper eye irrigation procedures to ensure resident safety and comfort. 

  • Conduct regular audits to ensure compliance with this policy and address any gaps in practice or documentation.
