Interdisciplinary Resident/Family Teaching

Goal: 

To maximize functional status and improve resident outcomes by promoting healthy behavior and involving the resident, and when appropriate, the family in care and care decisions. 

Policy: 

  • The Interdisciplinary Patient/Family Teaching form will be utilized for each educational need identified. 

  • The Interdisciplinary Team will determine the educational needs of each resident upon admission. 

  • As part of the ongoing care planning process for residents, educational needs will be identified. An educational goal will be determined, and approaches to meeting that goal will be developed. 

  • The Interdisciplinary Team will consider the learning needs of the individual resident and provide appropriate education to meet those needs. Printed materials, audio materials, and/or other types of educational materials may be used based on the individual resident’s needs. 

  • The facility supports resident education and will commit resources as necessary to meet educational objectives. 

  • All resident education will be documented by the discipline providing the education. 

Education Will Include the Following: 

  • Explaining the resident and family’s responsibilities in the resident’s care. 

  • How to safely and effectively use medication, when applicable, in accordance with the resident’s needs and legal requirements. 

  • How to safely and effectively use medical equipment, when applicable. 

  • Counseling on potential drug-food interactions, nutrition and hydration intervention, modified diets, and oral health, when applicable. 

  • How to use rehabilitation techniques to adapt to the environment or improve functional independence. 

  • Self-care activities, as appropriate. 

  • How to access available community resources, if needed. 

  • When and how to obtain further care or treatment after discharge. 

  • Discharge instructions given to the resident and those responsible for the resident’s continuing care. 

Procedure: 

Patient Education Assessment: 

  • Circle the appropriate answer to the following questions: 

  • Existing knowledge of disease/problem 

  • Cognitive barriers 

  • Attention span/memory 

  • Vision status 

  • Hearing status 

  • Communication 

  • Religious considerations 

  • Cultural considerations 

  • Physical limitations 

  • Learning style 

Educational Needs: 

  • Circle the appropriate answer. 

  • Anticipated date of discharge. 

  • Enter the appropriate code on the form for the following areas: 

  • Person taught 

  • Ready to learn 

  • Method of teaching 

  • Evaluation 

Documentation: 

  • The specific information taught and the signature and title of the discipline teaching will be documented.
