Feeding Tube Placement

PROBLEM:

The patient is at risk for complications due to feeding tube replacement.

GOAL:

The patient's tube will remain patent for the next 90 days.

PLAN/APPROACH:

  1. Elevate Bed Height:

    • Elevate the height of the bed by 30 - 45 degrees at all times.
    • Rationale: Elevating the bed reduces the risk of aspiration and reflux during feeding.
  2. Record Input and Output:

    • Keep a record of the patient's fluid intake and output.
    • Rationale: Monitoring fluid balance helps assess the patient's hydration status and overall well-being.
  3. Administer Medications via Tube:

    • Administer medications as ordered through the feeding tube.
    • Rationale: Administering medications via the tube ensures proper absorption and effectiveness.
  4. Monitor Labs:

    • Regularly monitor lab results as ordered by the physician.
    • Rationale: Lab monitoring helps evaluate the patient's nutritional status and identifies any imbalances.
  5. Monitor for Aspiration Symptoms:

    • Observe the patient for signs and symptoms of aspiration.
    • Rationale: Early detection of aspiration symptoms allows for prompt intervention and prevention of complications.
  6. Administer Free Water Flush:

    • Administer free water flush as ordered.
    • Rationale: Flushing the tube with water helps maintain its patency and prevents clogging.
  7. Check Tube Patency:

    • Check the tube for patency at least once per shift.
    • Rationale: Regular checks ensure that the tube remains unobstructed and functional.
  8. Monitor Tube Site for Infection:

    • Monitor the tube insertion site for signs and symptoms of infection.
    • Rationale: Early identification and management of infection prevent further complications.
  9. Treatment to Tube Site:

    • Provide treatment to the tube site as ordered by the physician.
    • Rationale: Proper care of the tube site minimizes the risk of infection and discomfort.
  10. Dietary Consult:

    • Consult a dietitian as needed to assess the patient's nutritional needs.
    • Rationale: A dietary consultation ensures that the patient's nutritional requirements are met.
