Resident Identification

Standard: 

To maintain a system that accurately identifies each resident. 

Policy: 

Each resident will preferably wear an identification bracelet. Resident photo will be available for additional identification. 

Procedure: 

I. Identification Bracelets: 

  • Identification bracelets will be placed on a resident’s wrist or ankle when necessary, on admission. 

  • Replacement bracelets will be made and placed on the resident’s wrist or ankle as needed. 

  • If a resident refuses to wear an identification bracelet, an alternate identification plan will be developed and documented in the resident’s care plan. 

II. Resident Photos: 

  • A photo of each resident will be taken upon admission. 

  • The photo will be kept in the resident’s medical record and accessible to staff for additional identification purposes. 

  • Photos will be updated as necessary to ensure accurate identification. 

III. Alternate Identification Plan: 

  • If a resident refuses to wear an identification bracelet, the interdisciplinary team will develop an alternate identification plan. 

  • The plan will be documented in the resident’s care plan and reviewed periodically. 

  • Alternative methods may include, but are not limited to: 

  • Marking personal items with the resident’s name. 

  • Utilizing room number and nameplates. 

  • Regular staff familiarization with resident identification through frequent interactions and care activities. 

IV. Documentation and Compliance: 

  • Staff will document the use of identification bracelets and any alternate identification methods in the resident’s care plan. 

  • Regular audits will be conducted to ensure compliance with the identification policy. 

  • Any issues or discrepancies will be addressed promptly to maintain accurate resident identification.
