Physician Orders – Licensed Practical Nurse

Standard

Telephone physician orders received by a Licensed Practical Nurse (LPN) must be evaluated and assessed by a Registered Nurse (RN) to ensure consistency with the current plan of care.

Procedure

When an LPN takes a new verbal order or an order change from a physician, the following steps must be followed:

I. Notification
The LPN must notify the RN immediately upon receiving a new verbal order.

II. Data Collection
The LPN may assist in collecting data pertinent to the verbal order. This data is then provided to the RN.

III. Verification and Documentation
The RN must:

  1. Review the new verbal order.
  2. Assess whether the order is consistent with the resident’s current plan of care.
  3. Sign their name and credentials under the physician’s order or in the electronic medical record, verifying that the order has been reviewed and is consistent with the resident’s plan of care.
