MDS Essentials: Tracking /Discharge MDS: Death in Facility

The Death in Facility Tracking Record, as outlined in the MDS 3.0 RAI Manual version 1.18.11 effective October 2023, is a crucial document for nursing facilities. This form is used under specific circumstances:

  1. When to Complete: It is required when a resident passes away within the facility or during a Leave of Absence (LOA).
  2. Completion Timeline: The form must be filled out within 7 days following the resident's death (as noted in the A2000 Discharge Date), and submitted within 14 days after the resident's death.
  3. Content: This tracking record includes demographic and administrative information about the deceased resident.
  4. Restrictions: It cannot be combined with any other type of assessment​​.

This process ensures that the facility maintains accurate and timely records of all resident deaths, adhering to regulatory requirements and supporting effective management of resident information.
