MDScare Nursing Home Cholecystostomy Tube Care Policy

Purpose: The purpose of this Cholecystostomy Tube Care Policy is to ensure the proper maintenance and monitoring of the cholecystostomy tube, including assessing tube patency and monitoring drainage, in order to promote resident comfort and well-being.

Definition: A cholecystostomy tube is a tube surgically inserted into the gallbladder for the removal of stones, bile, or pus. It is connected to a drainage bag or bottle.

Equipment:

  • Gloves (1 pair sterile, 1 pair non-sterile)
  • Sterile 4x4 gauze pads
  • Tape
  • Normal Saline
  • Any treatment ointments prescribed by the MD

Procedure:

A. General Considerations:

  1. The nursing staff will assess the cholecystostomy drainage tube during each shift to ensure patency and record the amount, consistency, and color of drainage.

  2. The insertion site will be monitored at least once during each shift for signs and symptoms of infection.

  3. Dressing changes to the insertion site will be conducted according to the physician's order, and the integrity of the insertion site's skin will be closely monitored.

  4. The drainage bag will be positioned at or below waist level and will be emptied by the nurse during each shift.

  5. Observations will be made for any color changes in the skin, sclerae (the white part of the eyes), urine, and stool.

  6. Vital signs will be taken during each shift.

B. Dressing Changes:

  1. Prepare the necessary equipment.

  2. Gently remove the old dressing and dispose of it, along with used gloves.

  3. Inspect the site for any abnormalities.

  4. Cleanse the site with normal saline.

  5. Apply any prescribed treatment ointment.

  6. Cover the site with a sterile dressing and secure it with tape.

  7. Ensure that tubing is securely taped to prevent dislodgment, while also maintaining patency and preventing obstruction.

  8. Keep the drainage bag positioned at or below waist level.
