Foley Catheter

PROBLEM:

Patient is at risk for complications due to Foley catheter use.

GOAL:

Patient will not have complications (e.g., UTI, obstruction) for the next 90 days.

PLAN/APPROACH:

  1. Educate the Patient on Aspects and Risks of Catheter Use:

    • Provide patient education about the purpose of the catheter, its proper use, and potential risks.
    • Rationale: Informed patients are more likely to cooperate, reducing the risk of complications.
  2. Keep Bag Below the Level of the Bladder:

    • Ensure that the catheter bag is positioned below the level of the bladder at all times to promote proper drainage.
    • Rationale: Maintaining proper bag positioning prevents urine reflux and potential complications.
  3. Anchor Tubing Securely to Avoid Drag and Monitor for Possible Twisting:

    • Securely anchor the catheter tubing to prevent tension and twisting.
    • Rationale: Preventing drag and twisting reduces the risk of trauma and catheter-related issues.
  4. Use Dignity Cover When Leaving Room:

    • Ensure that the catheter and drainage bag are covered appropriately when the patient leaves the room for privacy.
    • Rationale: Maintaining patient dignity and hygiene is important during catheter use.
  5. Monitor Urine for Cloudiness, Sediment, Hematuria, Odor; Notify MD as Appropriate:

    • Regularly observe the urine for any changes in color, clarity, odor, or presence of blood.
    • Rationale: Early detection of abnormal urine characteristics allows timely intervention to prevent complications.
  6. Monitor Catheter for Leaking, Clogging, and Poor Flow:

    • Routinely check the catheter system for leaks, clogs, and any decrease in urine flow.
    • Rationale: Detecting catheter-related issues early helps prevent urinary retention and other complications.
  7. Irrigate Foley as Ordered:

    • Perform catheter irrigation as prescribed by the healthcare provider.
    • Rationale: Irrigation helps maintain catheter patency and prevents obstructions.
  8. Obtain Specimens as Ordered:

    • Collect urine specimens for testing as directed by the physician.
    • Rationale: Specimen collection aids in monitoring urinary health and identifying potential complications.
  9. Encourage Fluids as Tolerated, If Contraindicated:

    • Encourage the patient to drink fluids within medical restrictions to maintain adequate hydration.
    • Rationale: Proper hydration supports urinary function and reduces the risk of infection.
  10. Monitor Intake and Output as Indicated:

    • Record and monitor the patient's fluid intake and urine output.
    • Rationale: Tracking intake and output helps assess urinary function and fluid balance.
  11. Catheter Care O Shift and PRN:

    • Perform catheter care during each shift and as needed to maintain hygiene and prevent infection.
    • Rationale: Regular catheter care reduces the risk of infection and promotes patient comfort.

Following this care plan, the healthcare team aims to manage the Foley catheter appropriately, minimize complications, and ensure the patient's comfort and well-being over the next 90 days. Continuous monitoring, education, and meticulous care contribute to successful catheter management.
