Goal: Residents will not leave the facility without a responsible person accompanying them.


Plan/Approach

  1. Continuous Monitoring:

    • Implement continuous monitoring of residents' whereabouts through regular checks and advanced tracking technologies (e.g., GPS-enabled devices).
  2. Escort Protocol:

    • Ensure residents are escorted to and from all activities and the dining room by trained staff members.
  3. Family Education:

    • Educate family members about the Leave of Absence (LOA) book, emphasizing the importance of signing in and out and the protocols in place to prevent elopement.
  4. Diversional Activities:

    • Encourage residents to participate in diversional activities tailored to their interests to reduce the urge to elope and enhance their engagement.
  5. Staff Awareness:

    • Conduct regular training sessions for all staff members to ensure they are aware of the residents' elopement risk and the protocols to follow.
  6. Reception Alert:

    • Alert receptionists about the residents' potential to elope and provide them with a clear protocol to follow if an attempt is suspected.
  7. Resident Identification:

    • Maintain a recent, clear photograph of the resident at the front desk and within the electronic health records for quick identification if needed.
  8. Wander Management System:

    • Utilize a wander management system, such as a wander guard bracelet. Ensure it is worn at all times and check its functionality regularly according to the manufacturer’s guidelines.
  9. Exit Security:

    • Monitor all exit doors, windows, and elevators to ensure they are secure and equipped with alarms or other deterrent systems.
  10. Redirection Techniques:

    • Train staff to effectively redirect residents who appear confused and are attempting to wander.
  11. Emergency Protocol:

    • Establish a clear protocol to notify authorities immediately if a resident successfully leaves the building unescorted.
  12. Risk Assessment:

    • Conduct elopement risk assessments upon admission and quarterly thereafter to identify changes in risk levels and adjust care plans accordingly.
