Policy 

To systematically track and document all resident transfers to acute hospitals, ensuring compliance with CMS Requirements of Participation and the State Operations Manual (SOM) guidelines for long-term care facilities (LTC). This tool aids in monitoring patterns, identifying trends, and enhancing the quality of care. 

Procedure 

  1. Documentation Requirements: 

  • Each resident transfer to the hospital must be documented using the Acute Hospitalization Tracking Tool.

  • The completed form should be reviewed monthly by the nursing supervisor and the Administrator to identify any patterns or trends that may require attention or intervention. 

  1. Data Usage: 

  • The information gathered from the tracking tool will be used to: 

  • Monitor and analyze hospitalization trends. 

  • Identify potential areas for improvement in resident care. 

  • Ensure timely communication and follow-up with hospitals. 

  • Assist in care planning and quality improvement initiatives. 

  1. Retention of Records: 

  • The completed tracking tools must be retained according to the facility’s record-keeping policies and in compliance with CMS regulations.
