Abdominal Binder, Application of

Application of Abdominal Binder 

Purpose: 

  • To provide even, firm support for the abdomen. 

  • To reduce pain. 

  • To hold dressings in place. 

  • To hold a gastrostomy tube in place. 

  • To hold ostomy pouches in place. 

Equipment: 

  • Abdominal binder of the type ordered. 

  • Fasteners (Velcro, tape, or pins as appropriate). 

Procedure: 

  1. Preparation: 

  • Verify the physician’s order for the use of an abdominal binder. 

  • Explain the procedure to the resident to ensure their understanding and cooperation. 

  1. Resident Positioning: 

  • Position the resident on their back, ensuring comfort and proper body alignment. 

  • Complete any necessary dressing changes or skin-care procedures. Cleanse and dry the area as needed. 

  1. Applying the Binder: 

  • Turn the resident onto their side, facing away from you. 

  • Roll the end of the binder towards the resident's midsection. 

  • Place the binder from the waist to the pubic line. 

  • Roll the resident to the opposite side and pull the ends of the binder free. 

  • Wrap the binder firmly across the abdomen and fasten it from bottom to top, using sufficient fasteners to maintain adequate support. 

  1. Securing the Binder: 

  • Ensure the binder is snug but not too tight, allowing for adequate support without restricting circulation or causing discomfort. 

  • Replace the top bedding, ensuring the resident is comfortably positioned with the call light within easy reach. 

  1. Monitoring: 

  • Check the placement and effectiveness of the binder at least once per shift. 

  • Observe for any signs of skin irritation, discomfort, or changes in circulation. Adjust the binder as necessary. 

  1. Documentation: 

  • Document the application of the binder, including the resident’s response, in the medical record. 

  • Note any observations related to the effectiveness and any adjustments made. 

Compliance and Documentation: 

  • Adhere to CMS guidelines and Requirements of Participation for Long-Term Care Facilities. 

  • Ensure that all procedures are documented accurately in the resident’s care plan. 

  • Regularly review and update the application techniques according to the latest clinical best practices and regulatory standards. 

  • Provide training to staff on the correct application and monitoring of abdominal binders to ensure resident safety and comfort.
