Ileal Conduit Care

Ileal Conduit Care 

Purpose: 

  • To preserve and maintain renal function. 

Equipment: 

  • The urinary appliance may consist of one or two pieces and may be disposable, semi-disposable, or reusable as ordered by the physician. 

Procedure: 

I. Preparation: 

  • The ileal conduit appliance is changed as ordered by the physician. 

  • The appliance should be changed early in the morning before taking fluids. 

  • Assemble all equipment needed for the type of appliance used. 

II. Removal: 

  • Moisten the edge of the face plate with adhesive solvent or soap and water. 

  • Gently remove the old appliance. Note: Adhesive solvent is not used if skin barriers (Reliaseal or Stomahesive) are used. 

  • Instruct the resident to bend over quickly and remain in that position for a minute to allow the conduit to empty before the skin is washed and dried. 

III. Cleaning: 

  • Clean all cement from the skin with adhesive solvent using a soft cloth. Pat dry. The skin must be dry or the appliance will not adhere. 

  • Inspect the skin for signs of irritation. 

  • Keep the skin free from direct contact with urine. 

  • A gauze or tissue wick may be applied over the stoma to absorb urine while the appliance is being changed. 

IV. Application: 

  • Prepare the appliance according to the manufacturer’s directions. The skin of the abdomen should be taut when putting on the appliance. 

  • Center the appliance directly over the stoma and apply it carefully. 

  • Apply gentle pressure around the appliance for secure adherence. 

  • Apply non-allergenic tapes in a picture frame effect around the pouch. 

  • The use of a belt is optional. If used, follow the manufacturer’s directions since an ill-fitting belt can cause abrasion to the stoma. 

V. Disposal and Documentation: 

  • Dispose of all disposable equipment. 

  • Document the change of the appliance, any irritation of the stoma, and any changes in urine color, etc., in the Narrative Nurses Note.
