Compress (Warm) 

Purpose: 

• To relieve inflammation and swelling. 

• To control pain. 

• To promote healing. 

Equipment: 

• Material for compresses (sterile, if appropriate). 

• Prescribed solution (sterile, if appropriate). Use water if no solution is prescribed. 

• Towels. 

• Container for solution (sterile, if appropriate). 

• Sterile gloves, if necessary. 

Procedure: 

1. Preparation: 

• Check the physician’s order for the type of solution prescribed, the area to be treated, and the length of time the treatment is to continue. 

• Gather all necessary equipment and ensure it is clean and sterile if required. 

2. Solution Preparation: 

• Moisten the compress material with the prescribed solution or water heated to no more than 110 degrees F. 

• Observe sterile technique if appropriate. 

3. Application: 

• Apply the compresses to the site to be treated for the time specified in the physician’s order. 

• Check the resident’s skin beneath the compress periodically for blanching, erythema, or any signs of adverse reactions. 

• Discontinue treatment and notify the physician if any adverse reactions occur. 

4. Post-Treatment Care: 

• When treatment is completed, check the resident’s skin. 

• Dry the treated area and apply a lubricant if appropriate. 

• Leave the resident in a comfortable position with the call light within reach. 

• Check the resident’s vital signs at the end of the treatment and document them. 

Compliance and Documentation: 

• Adhere to CMS guidelines and Requirements of Participation for Long-Term Care Facilities. 

• Document the application of warm compresses, including the resident’s response and any observations of skin condition, in their medical record. 

• Note any abnormalities or adverse reactions and actions taken. 

• Regularly review and update techniques for applying warm compresses according to the latest clinical best practices and regulatory standards. 

• Provide training to staff on proper warm compress application procedures to ensure resident safety and comfort. 

• Conduct regular audits to ensure compliance with this policy and address any gaps in practice or documentation.
