Residents are at risk for elopement, posing a potential safety concern.

GOAL:

Residents will not leave the facility without a responsible person accompanying them.

PLAN/APPROACH:

  1. Continuous Monitoring:

    • Staff will monitor the whereabouts of at-risk residents at all times.
    • Regular checks will help prevent residents from leaving unattended.

  2. Escorting Residents:

    • Staff will escort residents to and from activities and the dining room.
    • This ensures residents are always accompanied and safe.

  3. Family Education:

    • Families will be educated about elopement risks and prevention strategies through the LOA (Leave of Absence) book.
    • Understanding the risks empowers families to contribute to prevention.

  4. Engagement in Diversional Activities:

    • Residents will be encouraged to participate in engaging activities that reduce restlessness and wandering tendencies.

  5. Staff Awareness:

    • All staff members will be informed about residents at risk for elopement.
    • Staff awareness ensures consistent monitoring and prevention efforts.

  6. Reception Alert:

    • Receptionists will be alerted about residents' potential elopement risks.
    • This enhances vigilance and quick response.

  7. Photo Identification:

    • A recent photo of at-risk residents will be kept at the front desk.
    • This aids staff in recognizing residents and preventing unauthorized exits.

  8. Wander Guard Bracelet:

    • Residents will wear a wander guard bracelet at all times.
    • Proper functioning will be regularly checked based on manufacturer guidelines.

  9. Guidelines for Monitoring:

    • Exit doors, windows, and elevators will be closely monitored.
    • Enhanced surveillance helps prevent unauthorized exits.

  10. Managing Confusion and Wandering:

    • Residents exhibiting confusion and wandering tendencies will be redirected to safe areas.

  11. Immediate Reporting:

    • If a resident successfully leaves unescorted, authorities will be notified promptly.
    • Quick reporting aids in locating the resident and ensuring their safety.

  12. Regular Elopement Risk Assessment:

    • Elopement risk assessments will be conducted on admission and quarterly.
    • Regular assessments help identify changing risks and adjust preventive measures.
